Management of breakthrough pain question...

I have a friend that is on a regiment of medication for pain relief.
She takes on MSContin in the AM and one HS.  In addition she takes 2
tabs at noon.  For break through pain she takes MSIR prn.  The problem
is the with the breakthrough pain.  The MSIR just doesn't seem to cut
it.  It seems to me that if the MSC is not helping, the MSIR really
wouldn't do much better, even though it is an Instant Release v/s time
release.  I have no doubt that this person is in pain.  I have known
her all my life and trust her implicitly.  I know this also seems like
a lot of pain meds, and for  me, it would be, but she is able to
function when her pain is not too severe.
I was wondering if anyone has a suggestion for break though pain that
might work.  I was thinking of actiq but the doctor thinks that the
problem is managing the chronic pain not the breakthrough and "Actiq
is for acute pain."
I have also tried every non pharmalogical pain relief method I know
with only occasional success, albeit slight

So, if you have any suggestions I'd be thankful to hear them.
And, I'm sure I"ll get my share of flames along with the helpful
responses, but that it the nature of the beast.
